To order the test, you can either download the Test Requisition Form (TRF) and email us the signed form, or fill it out electronically using DocuSign.

Please make sure it is also printed and included with the specimen before shipping.

Patient Consent Form

You can download the patient consent form, and can email or include the signed form with the shipment.
