Ordering GenoPredicta is straightforward. Complete the Test Requisition Form (TRF) with the required clinical and patient information, then follow the kit and shipping instructions below to collect and send your sample using either peripheral blood or bone marrow. Everything you need to order, consent, and ship is on this page – and our team is here to help if you have questions along the way.
Test Requisition Form (TRF)
Required to order the GenoPredicta test, capturing essential clinical details and patient information to ensure accurate processing and reporting.
Instructions:
- The GenoPredicta Test must be ordered by a licensed Medical Provider through a Test Requisition Form (TRF).
- The combined TRF and patient consent forms are available here and are also included with the GenoPredicta kit.
- A completed TRF must accompany all samples/kits. Completed consent forms can also be emailed or faxed to (857) 855-5272. Licensed medical professionals may also submit TRFs online via DocuSign.

- Testing services are currently available in all U.S. states except New York. We are actively working to bring access to these locations. To be notified when testing becomes available in your state, please contact us.
